TERMS & CONDITIONS / CONTRACT
Bay Therapy
TERMS & CONDITIONS / CONTRACT
This is a mutual agreement negotiated between your psychologist and the client prior to the commencement of our 50 minute sessions. It articulates the responsibilities of your psychologist and client in the context of the therapeutic relationship they are going to undertake together.
CONFIDENTIALITY
This is a key aspect of the relationship between both the client and your psychologist. Everything discussed in the sessions is kept in the strictest confidence. Your psychologist has the responsibility and commitment to work under regular supervision. From time to time she may need to share and discuss some of the information in the sessions with a qualified supervisor, always under a strict confidential and professional framework. The client's identity is always kept anonymous.
Confidentiality can only be broken when there is evidence that the client may harm him/ herself or others. In this case your psychologist will most likely contact the appropriate services. The client will be informed in advance, and only in case of an imminent threat might your psychologist make this decision without consulting the client. Please inform your psychologist at the beginning of the sessions of any medical treatment you might be following due to emotional difficulties.
FEES
£160 per session Initial session
£640 payable as a block of 4 sessions if you decide to proceed with your therapy plan after your initial assessment. This payment amount is due 72 working hours prior to your therapy intervention.
After session 4, you will have the option to pay per session at a fee of £160 or proceed with another 4 session block of £640, depending how many estimated sessions your Dr has advised.
WHEN WILL MY SESSION TAKE PLACE?
Our Clinic Director will be able to provide you with their availability and will aim to offer you the quickest appointment possible. As it is a private service, we do have extremely busy periods and you may have to wait a little longer than expected, but we will do the best we can to offer you an appointment as soon as possible.
When is my payment due?
Payment for your initial session is to be paid for on the same working day as booking your appointment.
Appointments after your initial appointment are to be paid 72 working hours in advance of your next appointment payable via BACS. No card facilities, cash or cheques. Please note any payments that are not made on time will automatically be cancelled.
INSURANCE -
When using insurance it can take from 2- 4 weeks to set up with our checks and admin. When your insurance policy is live, if you are unable to attend your session within the cancellation period (72 working hours) your full session will still be claimed from your insurance under our terms and conditions. If you still require sessions with Dr Symonds and your sessions have come to an end with your insurance provider, you may pay £160 per session to continue. If circumstances change with your policy and you do not put it in writing to us, you may be liable for money owing if we are unaware.
CANCELLATIONS / HOLIDAYS / SICKNESS
Cancellation of sessions with less than 72 working hours notice will incur the full fee.
Your psychologist will always aim to give the client as much notice as possible of any holidays, training workshops, conferences or illness that might prevent them from being available at the time/day of the scheduled session. In such cases the Psychologist will always aim to offer an alternative arrangement. Sessions will not take place if the client is under the influence of any misuse of alcohol or substances.
DURATION OF THE SESSION
Please note your Appointment is a strict 50 minute Appointment.
NO VERBAL ABUSE
Please note that verbal abuse will not be tolerated by any measure. This is by the form of email or verbal. We reserve the right to decline therapeutic intervention should we receive any verbal abuse by anyone in your party at any time. If you have already begun your therapeutic intervention with us and we feel you have compromised the relationship by your actions of verbal abuse, your therapy will be terminated immediately and any fees paid or due will be at a loss.
INDIVIDUAL RISK - CHILD & ADULT
Please note that we are not a crisis service. Upon your enquiry, it is extremely important to report any form of self harm or active suicidal plans / suicidal thoughts to our Clinic Director, either verbally or email. If this is not reported and these details are picked up within the initial assessment, it may be that our clinical psychologists feel that you fall outside of our remit and you would be signposted to a crisis service immediately.
END OF THE CONTRACT
Indications and signs that the therapeutic process is coming to an end: recovered inner balance, greater confidence in oneself, capacity to make decisions and face situations when previously feeling overwhelmed, being in touch with one's own resources and strength. The client may not have met all the goals at this point, though feels more confident to achieve them individually, and overall the individual's well being has increased. Sometimes the client may feel that therapy is not helping. In these circumstances it is best to discuss the difficulties rather than abruptly end their sessions. This could evoke a sudden-loss experience that would not have the opportunity to be understood and resolved. In such circumstances your psychologist asks that the client give one week's notice before ending their sessions to have the chance to discuss the decision and to complete the process adequately. The client is always involved in the decision to continue or stop therapy and will not be under any pressure to continue at any point.
CONTACT OUTSIDE THE SESSIONS
The phone number and email address provided are to be used exclusively for cancellations or to re-arrange your appointment. Please note that messages will be monitored regularly but not continuously. Therefore, in case of an emergency you are advised to call the appropriate emergency services, such as the hospital, dial for an ambulance and/or if appropriate contact your GP. https://www.elysianpsychology.co.uk/crisis-page Our Clinic Director will endeavour to reply to all messages as soon as possible but only within her clinic hours.
LETTER FEES
Please note that we do charge a fee for letters requested for schools/ universities or other areas. However, please speak to your psychologist regarding your requirements and our Clinic Director will provide you with a cost. This fee must be paid in full prior to any letter being written and we must have it in writing that you are happy to pay this fee before your psychologist will write an agreeable letter for you.
TELEPHONE CONSULTATIONS
We understand that from time to time you may wish to have a discussion with your psychologist regarding certain matters, however please note that there is limited available time due to busy clinic sessions. However your psychologist will do her very best to ensure they can offer a telephone consultation with you. To ensure you get the most out of your telephone consultation with your psychologist, there is a minimum fee of £75 per 30 minutes. Please note that this payment is payable in advance and there is a cancellation fee of 24 hours should you no longer need this telephone consultation.
DATA PROTECTION & GDPR
We aim to be as clear as possible about how and why we use information about you/your child, so that you can be confident that your privacy is protected. This policy describes how we manage your information when you use our services.
This policy describes the information that we collect when you use our services. This information includes personal information (data) as defined in the General Data Protection Regulation (GDPR)
The policy describes how we manage your information when you use our services, if you contact us or when we contact you. It also provides extra details to accompany specific statements about privacy that you may see when you use our website (such as cookies). If you visit our website, information can be found regarding the use of cookies in the Cookie Policy. This outlines information about the type of cookies that our website uses and how you may disable those cookies, if you choose to do so.
We use the information we collect in accordance with all laws concerning the protection of personal data, including the Data Protection Act 1998 and the GDPR 2016. As per these laws, we are the data controller for the information that you give to us. Our contact details can be found on my website https://www.dr-annasymonds.co.uk/ or https://www.elysianpsychology.co.uk/
If another party (eg CAMHS, the GP) has requested access to your data we will ask for your consent to share information and outline who they are, what they will do with your data and why we need to provide them with the information. There are two legal bases where we do not need to ask for your consent to share the data with a third party: if there is a risk to your or your child’s vital interests or if we have a legal obligation to share the data.
The GDPR requires that we identify the legal basis upon which we process your personal data. I shall provide psychological services and all associated activities on the basis of:
⦁ Our contract with you
⦁ My legitimate interest to hold and process your personal data so that we can determine whether it is appropriate for me to provide services to you.
What is meant by ‘The contract’?
When an assessment is requested, the client is asked to read and sign our terms and conditions. That document describes the work that we will undertake and, by signing the document, you enter into a contract with us to do the work. We will process all personal data that you share with us (for the purpose of completing an assessment) lawfully, fairly and in a transparent manner. It will be necessary for us to process your personal data in order to fulfil the contract with you.
What are my ‘legitimate interests’?
The reason that we need to process your personal data is to provide psychological services to you/your child. Assessments/consultations/Therapy involve the processing of special category data, including information, for example, about your child’s health, cognitive functioning, social and emotional issues and family history. We have a legitimate interest to collect such personal data for the purpose of forming a well-informed professional opinion. We will only collect information from you that is relevant for the process of providing the psychological services that you have requested.
Why do we need to collect your personal data?
In order to carry out an effective assessment or consultation, we need to collect information about you and / or your child so that we may:
⦁ Communicate with you by email or phone. The legal basis for this is a legitimate interest.
⦁ Deliver services to you/your child, for example by preparing a relevant assessment and providing well informed psychological advice for your child. The legal basis for this is the contract with you.
What personal information do we collect and when do we collect it?
To provide helpful psychological services to you/your child, it is important for us to know you/your child’s situation and what the concerns are, that led to psychological support being requested. This data is usually collected by the adult / parent or primary carer (if on behalf of a child). Some parents may choose to provide additional information in writing or verbally.
How do we use the information that we collect?
The information collected is used in the following ways:
⦁ To communicate with you so that we can arrange appointments with you, discuss your concerns and arrange an appropriate course of action.
⦁ To prepare a relevant assessment / consultation for you/your child. I need the personal data to help us to decide if we are a suitable service for you.
⦁ Where do I keep the information?
⦁ When data is received by email, attachments will be saved in the individual’s electronic file.
⦁ We have a secure electronic system whereby your file is stored and personal data is locked.
⦁ How long do we keep the information?
⦁ Electronic files will be retained for a period of six years.
⦁ After a period of six years all remaining records will be securely destroyed, unless we continue to be directly involved with you/your child.
⦁ Will we send emails and text messages to you?
⦁ We will send you emails and text messages to serve as reminders of your Appointments or to serve you with details of your Appointment which we feel is necessary.
